How Much Do Medical Couriers Make? Pay and Costs

Why medical delivery pays above standard courier work — and what it demands in return

By the Monthly Income Booster editorial team · Updated · How we research and fact-check

Medical courier work — lab samples, pharmacy deliveries, medical records — generally pays better than food delivery because the requirements are stricter and the schedules are fixed.

What BLS data shows (May 2025)

The U.S. Bureau of Labor Statistics' Occupational Employment and Wage Statistics survey (May 2025 national estimates) puts the median wage for couriers and messengers (SOC 43-5021) at $18.85 an hour, or $39,200 a year. The lowest-paid 10% earned under $15.17 an hour and the top 10% more than $24.20. Couriers and messengers on a payroll. Independent medical couriers using their own vehicle are contractors and are not in the survey; their vehicle costs also come out of their pay.

Routes Versus On-Demand

Scheduled routes give predictable daily income and predictable mileage. On-demand and STAT runs pay more per trip but leave gaps in the day.

The Costs Are Real

Fuel, maintenance, insurance suited to commercial use, and sometimes coolers or approved containers. Most couriers are contractors, so those costs come out of the rate.

Requirements

A reliable vehicle, a clean driving record, background checks, and often specific handling training. Some contracts require particular insurance before you can start.

Check Your Real Hourly

Use the mileage deduction and gig pay calculators to see what a route leaves after fuel and vehicle wear.
